こちらのフォームから、「第56回日本結合組織学会学術大会」の演題申込を受け付けます。

【お申し込みの流れ】

  1. 以下のフォームに必要事項をご入力の上、［確認］ボタンを押してください。
  2. 確認画面が表示されますので、入力内容をご確認の上、問題が無い場合は［送信］ボタンを押してください。
  3. 「エントリーを受け付けました」という登録完了画面が表示され、ご登録のメールアドレス宛に自動返信メールが送信されます。

【申込に関する注意事項】

  2. WEB登録が完了しましたら確認メールが自動送信されます。
    翌日になっても確認メールが届かない場合はご連絡ください。
  3. 申込内容の変更・取消は、締切り前であれば自由にWEBシステム上で可能です。
    締切後の変更はWEB上では一切できませんのでご注意ください。
  4. 本フォームを、ブラウザの複数のタブに同時に表示させて登録することは、登録エラーの原因となりますのでお控えください。
  5. 環境依存文字のご使用はお控えください。
  6. 「Gmail」「Yahoo!メール」「Outlook.com（Hotmail）」などのフリーメールを使用したご登録は、自動返信メールが届かない可能性がございますのでお控えください。
  7. セキュリティ上の理由により、本フォームを開いてから40分以内で登録を完了してください。
    40分を超えると、入力した内容が破棄されたり、エラーが表示されます。
    登録内容の修正時も同様にお願いいたします。
